THE EAST HARLEM
LUPUS COOPERATIVE

A Collaborative Program for the Management of Chronic Illness among Young Inner-city Minority Women.

The East Harlem Lupus Cooperative is a joint program of the SLE (Lupus) Foundation and the East Harlem Lupus Community Task Force and Professional Advisory Committee. The new community-based project will create an integrated network of existing professional and community services to coordinate and manage the ongoing care of people with Lupus. The Cooperative will improve the quality, availability and delivery of medical and support services of women with Lupus and their families, and demonstrate a new collaborative model for the effective management of chronic illness among young inner-city minority women.

The East Harlem Lupus Cooperative will:

  • Raise community awareness through new untraditional and culturally appropriate outreach and education programs.

  • Recruit and convene a community partnership of existing medical and service providers and advocates to form a functioning cooperative organization.

  • Facilitate access to screeing, diagnosis, and treatment for women at risk for Lupus.

  • Coordinate the mix of ongoing medical, social, and financial support services needed by chronically ill Lupus patients and their families.
